social support to seniors who live at home but are
at risk of decline and admission to hospital or a
residential care center if support is not provided.
Eligibility
For Home Health Care services, clients must meet
standard provincial health care eligibility criteria

guidelines for Calcium
and Vitamin D
• Having regular
vision check-ups and
correcting vision
problems
• Reviewing your

outdoors
