Call PAM

(Patient Attachment Mechanism)

From Chilliwack to Boston Bar, any com-

munity member without a family doctor

or nurse practitioner who wants one can

just “Call PAM”. Depending on where you

live, you may be directly matched to a

family doctor or nurse practitioner in your

community, or attached to the Chilliwack

Primary Care Clinic (CPCC) where you will

receive care until you are ready for transi-

tion to a community family practice. Some

patients with complex conditions may

continue to receive ongoing services from

Primary Care Seniors Clinic

The Primary Care Seniors Clinic is accessed

through a referral from your family doctor.

It offers assessment, diagnosis and indi-

vidualized care planning for patients with

dementia concerns or multiple chronic

conditions and complex health issues. A

team of health care professionals including

primary care doctors, a nurse specializing

in seniors care, a geriatrician and geriatric

psychiatrist work alongside patients, their

family members and family doctors to en-

sure seniors are supported in their ongo-

ing health needs.

Residential Care Program

For the best care of the frail elderly, spe-

cifically those in residential care, family

doctors have a pivotal role in defining, sup-

porting and leading quality of care. This

includes complex patients whose health

challenges impact cognition and behav-

ior, and those that have chronic, long-term

disease impacts. The Residential Care Pro-

gram allows family doctors the ability to

provide an effective and efficient method

for the delivery of comprehensive and high

quality care to patients. Residential care

remains an important part of full service

family practice and the Chilliwack Division

remains committed to assisting in the pro-

vision of that care.

The Chilliwack Division of Family Practice is a non-profit society that represents

family doctors and nurse practitioners in Chilliwack, Agassiz-Harrison, Seabird

Island and Hope. Here are some of the initiatives the Chilliwack Division is

working on to help improve primary health care in our communities.
